To apply for new utilities services in the Town of Chapin, please bring the following documents to the Town of Chapin Utilities Office:
- Completed Application for Utilities Service
- Completed Sewer Service Agreement
- Completed Declaration of Covenant
- Copy of current lease or closing statement
- Form of payment for required fee
- Valid driver's license
Please print out all forms on one-side of 8.5" x 11" paper (no duplex please).
